Once it has
opened, place your cursor over the line in which you wish to type.
Type the
information needed and tab or move your cursor to the next line.
Once the
form is complete, print and submit as instructed on the form.
If you can
not type directly into the form, it has not been set to allow
text fields. You will need to print the form and fill it out by
hand. Please email the webmaster, and we will update the form
to allow direct typing in the future.
